目的 分析早期介入丹佛模式(ESDM)康复行为训练对孤独症谱系障碍(ASD)患儿症状、社会适应及情绪障碍的改善效果。方法 选取2023年1—11月金华市妇幼保健院收治的60例ASD患儿为研究对象,按照随机数字表法分为对照组30例和研究组30例。对照组患儿进行康复行为训练,研究组在对照组基础上进行ESDM康复行为训练。观察两组ASD症状、社会适应能力、心理状态、ASD程度、行为能力及神经发育水平。结果 治疗后,两组孤独症治疗评估量表(ATEC)、幼儿儿科症状检查表(PPSC)、孤独症评定量表(CARS)、孤独症行为量表(ABC)评分均降低(均P<0.05)。治疗后,研究组患儿ATEC量表、PPSC量表评分均明显低于对照组(均P<0.05),研究组CARS、ABC量表评分分别为(21.54±1.97)分、(54.14±6.23)分,明显低于对照组的(38.25±4.01)分、(64.25±7.25)分,差异均有统计学意义(t=20.486,5.793,均P<0.05)。治疗后,两组婴儿-初中学生社会生活能力量表(S-M)、Gesell发育诊断量表评分均升高,且研究组均高于对照组(均P<0.05)。研究组儿童家长对医护的满意度高于对照组(P<0.05)。结论 ESDM康复行为训练可降低ASD患儿ATEC量表评分,提高患儿社会适应能力,改善情绪障碍、ASD症状,效果较好。
Abstract:Objective To analyze the improvement effect of early start Denver model(ESDM) rehabilitation behavior training on symptoms, social adaptation, and emotional disorder in children with autism spectrum disorder(ASD).Methods Sixty children with ASD admitted to Jinhua Municipal Maternal and Child Health Care Hospital from January 2023 to November 2023 were selected as research subjects. They were divided into control group(30 cases) and study group(30 cases) according to random number table. The children in control group received rehabilitation behavior training, while the children in study group received ESDM rehabilitation behavior training on the basis of control group. The symptoms, social adaptation abilities, psychological states, degrees of ASD, behavioral abilities, and neurodevelopmental levels of the two groups were observed. Results After treatment, the scores of Autism Treatment Assessment Scale(ATEC), Preschool Pediatric Symptom Checklist(PPSC), Autism Rating Scale(CARS), and Autism Behavior Scale(ABC) decreased in the two groups(P<0.05). After treatment, the scores of ATEC scale and PPSC scale in study group were significantly lower than those in control group(P<0.05), the scores of CARS scale and ABC scale in study group were(21.54±1.97) and(54.14±6.23), respectively, which were significantly lower than those in control group [(38.25±4.01) and(64.25±7.25), respectively](t=20.486, 5.793, P<0.05). After treatment, the scores of Normal Development of Social Skills from Infant to Junior High School Children(S-M) and Gesell developmental diagnostic scale scores in the two groups increased, which were higher in study group(P<0.05). The satisfaction in study group was higher than that in control group(P<0.05). Conclusion ESDM rehabilitation behavior training can reduce ATEC scale score, improve social adaptability, improve emotional disorders and ASD symptoms of children with ASD, the effect is good.
