血清miR-9 miR-141及miR-320表达水平对妊娠期急性胰腺炎患者病情严重程度的预测价值

摘要:

目的 分析血清微小核糖核酸(miR)-9、miR-141、miR-320表达水平对妊娠期急性胰腺炎患者病情严重程度的预测价值。方法 选取2022年1月—2024年1月在丽水市人民医院收治的妊娠期急性胰腺炎患者92例,根据病情程度分为轻度组(60例)和重度组(例32),另选同期进行孕检的健康孕妇88例为对照组。qRT-PCR法检测血清miR-9、miR-141、miR-320相对表达量,采用多因素logistic回归分析患者病情程度的影响因素,绘制受试者工作特征(ROC)曲线分析血清miR-9、miR-141及miR-320对患者病情程度的预测价值。结果 患病组的血清miR-9(0.72±0.09)、miR-141(0.81±0.09)、miR-320(0.76±0.08)水平显著低于对照组(1.01±0.11,1.01±0.12,1.02±0.11,P<0.05);重度组的血清miR-9(0.63±0.08)、miR-141(0.74±0.08)、miR-320(0.68±0.07)水平显著低于轻度组(0.77±0.09,0.85±0.09,0.80±0.06,P<0.05),BISAP(4.12±0.43)分、APACHEII评分(12.68±1.46)分显著高于轻度组(3.35±0.37分,10.25±1.38分,P<0.05); miR-9、miR-141、miR-320均为影响患者病情程度的保护因素(P<0.05); miR-9、miR-141、miR-320以及三者联合预测患者病情程度的AUC分别为0.876、0.859、0.828、0.951,联合预测的AUC显著优于miR-9(Z=2.393,P<0.05)、miR-141(Z=2.725,P<0.05)、miR-320(Z=2.872,P<0.05)单独预测。结论 妊娠期急性胰腺炎患者血清miR-9、miR-141、miR-320水平降低,对妊娠期急性胰腺炎病情严重程度具有一定辅助预测价值。

Abstract:

Objective To study and analyze the predictive value of serum miR-9,miR-141 and miR-320 expression levels for the severity of acute pancreatitis during pregnancy. Methods A total of 92 patients with acute pancreatitis during pregnancy admitted to Lishui People's Hospital from January 2022 to January 2024 were selected. They were grouped into mild group(n = 60) and severe group(n = 32)according to the severity of disease. Additionally,88 healthy pregnant women who underwent prenatal examinations during the same period were collected as the control group QRT-PCR method was applied to detect the relative expression levels of serum miR-9,miR-141 and miR-320. Multivariate logistic regression was applied to analyze the influencing factors of severity of disease. Receiver Operating Characteristic(ROC) curve was plotted to analyze the predictive value of serum miR-9,miR-141 and miR-320 for the severity of disease.Results Compared with the control group,the serum levels of miR-9(0. 72±0. 09),miR-141(0. 81±0. 09),and miR-320(0. 76±0. 08) were obviously reduced in the diseased group than in the control group(1. 01±0. 11,1. 01±0. 12,1. 02±0. 11,P<0. 05). The serum levels of miR-9(0. 63±0. 08),miR-141(0. 74 ± 0. 08) and miR-320(0. 68 ± 0. 07) were obviously reduced in the severe group than the mild group(0. 77±0. 09,0. 85±0. 09,0. 80±0. 06,P<0. 05),and the BISAP score(4. 12±0. 43) points and APACHEII score(12. 68±1. 46) points were obviously increased than the mild group(3. 35±0. 37 points,10. 25±1. 38 points,P<0. 05). MiR-9,miR-141,and miR-320 were all protective factors that affected the severity of disease(P<0. 05). The AUC of miR-9,miR-141,miR-320,and their combination in the prediction of severity of disease was 0. 876,0. 859,0. 828,and 0. 951,respectively. The AUC of combined prediction was obviously better than that of miR-9(Z = 2. 393,P<0. 05),miR-141(Z = 2. 725,P<0. 05),and miR-320(Z = 2. 872,P<0. 05) alone. Conclusion The serum levels of miR-9,miR-141,and miR-320 in patients with acute pancreatitis during pregnancy decrease,which has certain auxiliary predictive value for the severity of acute pancreatitis during pregnancy.
