孤独症谱系障碍与语言发育迟缓患儿电子屏幕暴露及亲子互动情况研究

摘要(Abstract):

目的 探讨孤独症谱系障碍(ASD)与语言发育迟缓患儿电子屏幕暴露及亲子互动情况。方法 选取2020年6—12月就诊于连云港市妇幼保健院的4岁以内ASD(包含早期非典型ASD)和语言发育迟缓患儿各64例,分别为ASD组和语言发育迟缓组,选取52例健康体检儿童为对照组,按照<1岁、1~1.5岁、>1.5岁分为3组。采用自编调查问卷对儿童主要抚养人进行有关电子屏幕暴露及亲子互动内容的调查。分别对比ASD组、语言发育迟缓组与对照组间,以及两病例组间儿童电子屏幕暴露及亲子互动情况的差异。结果 与对照组儿童相比,ASD组及语言发育迟缓组儿童的首次电子屏幕接触时间、日累计接触时间、频繁接触时间跨度、家长是否讲解方面差异均有统计学意义(均P<0.05)。3组儿童首次及频繁接触的电子屏幕种类均为电视机最多,其次为手机、IPAD、电脑等。ASD组较对照组频繁电子屏幕接触的起始年龄早[(16.21±7.17)个月、(19.54±6.96)个月,t=2.31,P<0.05]。ASD组较语言发育迟缓组首次电子屏幕接触时间更早,接触时家长讲解更少(χ~2=9.06,4.46,均P<0.05)。与对照组儿童相比,ASD组与语言发育迟缓组亲子互动时间及与主要抚养人互动时间均较少,互动内容在2项及以上的比例也较低(均P<0.05)。结论 ASD及语言发育迟缓儿童存在电子屏幕暴露和亲子互动问题。相比语言发育迟缓儿童,ASD患儿的电子屏幕暴露时间更早,家长讲解更少。与对照组儿童相比,ASD组、语言发育迟缓组儿童亲子互动时间及内容均较少。因此,在婴幼儿早期养育中对家长实施电子产品使用的科普教育以及加强亲子互动宣教非常必要。
